Alternatively you can download an e-form HERE (a pdf reader is required) and return by email or post or you can call at the school to collect one.

Please complete the form by clicking on each field and typing, then save and email it back to us, or you can print the form to complete by hand then return to us by post or by handing into reception.

Please note the following dates for our assessment test are for December Testing : Thursday 13th December at 4.15 p.m. and Saturday 15th December at 9.30 a.m.

It is a requirement of our Admissions Policy that all children must sit our assessment test.

This is not a pass or fail test – but ensures that all children regardless of what ability level they are are admitted to the school.

Please look at the Local Authority details on line via www.liverpool.gov.uk  You need to complete the local authority preference form.   You have to list your preferences for a secondary school for your child.  If you wish your child to be considered for a place at North Liverpool Academy you must put us down as your 1st choice.
